Process for Surveys/Questionnaires

When sending surveys or questionnaires to the entire student population or a sample of the student population, the survey/questionnaire must go through administrative review. View the University of Iowa Operations Manual and University Human Resources pages for more information on this process. The Administrative Review Panel, composed of the University-wide officers, approves any non-collegiate surveys/questionnaires.

The purpose of this panel is to consider the institution's need for information in order to be accountable and the concerns for individual privacy. In addition, this procedure provides a more comprehensive view of the many information gathering activities of the University and provides a means for systematic administrative review of such activities.

Review Levels

Level Description Feedback Timeline
1 Typically involves an annual survey/questionnaire that has been approved as a level2 or level3 survey/questionnaire in a previous year. No feedback is requested from the review panel. 3 working days
2 Typically involves a new survey/questionnaire that the co-chairs have evaluated as straightforward and approved. The co-chairs shall send an announcement with the survey/questionnaire instrument requesting feedback within 5 working days. 5 working days
3 Typically involves a new survey/questionnaire that the co-chairs have evaluated as raising issues for discussion either in content or format. The co-chairs shall send an announcement with the survey/questionnaire instrument requiring feedback within 10 working days. Feedback at Level3 is sent directly to the co-chairs. 10 working days

Implementation Guidelines

The Associate Director of Assessment, Improvement, and Research will facilitate the administrative review process for the Division of Student Life. The following materials should be sent to the associate director.

  • A cover letter stating the goals of the survey/questionnaire
  • The survey or questionnaire
  • The desired targeted audience
  • Anticipated distribution date
  • Any cover letter, instructions, or other material that will accompany the survey or questionnaire when sent to the targeted audience

These materials will be submitted by the Associate Director of Assessment, Improvement, and Research to the Associate Provost for Faculty and the Associate Vice President and Director of Human Resources who serve as review panel co-chairs.
